Delivering exceptional care at home for complex needs in the community
Working in partnership with NHS North of England Commissioning Support (NECS), Thornbury Community Services (TCS), part of Acacium Group, has enabled Ronan, a young man with complex learning disabilities and mental health needs, to live safely and independently in his community.
Highlights
- 17,520 hours of care delivered every year
- 2:1 support, 24/7 with waking nights
- Fully funded, ongoing, person-centred care
The challenge
Ronan previously lived at home with his parents and siblings, all of whom had additional needs. The environment was overstimulating and unsuitable for his complex requirements. Ronan struggled with communication, displayed frequent physical aggression and often withdrew from family life, sleeping for long periods and avoiding interaction. His behaviour posed a risk to himself and his family and his quality of life was significantly impacted.
Why Acacium Group
Thornbury Community Services, part of Acacium Group was uniquely positioned to support Ronan due to its specialist learning disabilities, autism and mental health care at home service. With over 20 years of clinical excellence and an ‘Outstanding’ CQC rating, TCS offers a fully managed, person-centred approach backed by robust clinical governance.
The solution
TCS began by carrying out a thorough assessment of Ronan’s needs, working closely with his family and commissioning partners to design a bespoke care plan. A dedicated 2:1 care team, carefully matched to Ronan, was mobilised to provide 24/7 support, including waking nights. The team established a calm, structured environment tailored to Ronan’s preferences, which helped reduce his agitation and aggression and created a safer, more stable home life.
As trust developed, the team focused on building positive relationships and encouraging Ronan’s communication and independence. Led by Chief Nurse Clare Metcalfe and overseen by qualified learning disability and mental health nurses, the service is underpinned by robust clinical governance and regular reviews. With a fully funded, ongoing care package, Ronan continues to receive high-quality, person-centred support for as long as he needs it, giving reassurance to both his family and commissioners.
