Xyla , part of Acacium Group has delivered the Integrated Healthier Futures Programme in East Riding of Yorkshire since April 2023. The service aims to connect, empower, motivate, and assist residents in positively changing their lifestyle behaviours.

The challenge

East Riding, characterised by its rural landscape and coastal regions, faces unique challenges in healthcare accessibility. The area’s geographical spread can make it difficult for residents to access services readily, particularly in more isolated communities.

Deployed in a phased approach, the programme was launched by thoroughly assessing community health needs. The results informed the development of a comprehensive strategy to address key health topics like nutrition, habit formation, and the risks associated with alcohol and substance use.

Xyla’s multidisciplinary team offered holistic health services by leveraging innovative technology and proven behavioural change models. The programme blended behavioural change strategies, personalised support, and innovative digital resources, including a bespoke app and website, to ensure accessible and continuous support.

The Healthier Future Programme’s effectiveness was continuously monitored and evaluated to address the unique health challenges and socioeconomic factors in East Riding, with ongoing refinements enhancing its impact on reducing health disparities in the region.

Impact

Community engagement

The service successfully processed 2,186 referrals, indicating a high level of community engagement and outreach. These referrals were generated through various channels including self-referral, primary care, secondary care and social prescribers.

Improved health outcomes

The programme notably surpassed its key performance indicators (KPIs), with over 75% of participants completing the Adult Weight Management programme achieving weight loss, and more than 55% of individuals in the smoking cessation initiative successfully quitting within four weeks.

Reduced health inequalities

The Quit-Smoking programme demonstrated significant impact and wide-reaching appeal, engaging a diverse demographic: 30% non-white British, 35% aged over 60, and 50% economically inactive, showing its success across various age groups, economic statuses, and genders, with participation from 64% female, 35% male, and 1% non-binary individuals.
