This form gives us your permission to include you (or the person you are signing on behalf of) in that content, now and in the future. It helps us manage this process with transparency and respect.

By completing this form, you confirm that:

1. You give Acacium Group (and its Group Companies) permission to record and use your image, voice, likeness, name, organisation (if applicable) and any quotes you provide.
2. This applies to content captured today and any future content you choose to participate in, whether through video, photography, audio recordings, written contributions, or similar formats.
3. Your content may be used across a variety of channels, including but not limited to:
   • Internal communications (e.g., presentations, training, newsletters)
   • Company websites and social media
   • Marketing and promotional materials
   • Press and media coverage
   • Events, webinars and reports
4. You grant Acacium Group (and its Group Companies) the right to use this content globally, in any format, for an unlimited time.
5. You will not seek compensation or assert privacy, publicity, or copyright claims related to its use.
6. You confirm that you are over 18 years of age and have the authority to sign this release (including on behalf of a minor or dependent, if relevant) and understand and agree to these terms.
