The community and complex care we offer
Complex care
We deliver specialised care to individuals with intricate clinical and social needs. No matter their condition’s complexity, we support patients and service users to get the care they need at home.
Benefit from a team with expertise capable of supporting a wide range of complex conditions, including acquired brain injury, end-of-life care, neurological conditions, palliative care, physical disabilities, spinal injuries, and mental health. We are committed to delivering tailored care that meets the unique demands of each patient, ensuring the highest quality of life and wellbeing for those we support.
Mental health
Reduce the demand for inpatient beds and the number of hospital readmissions for individuals living with complex mental health conditions. Our intensive home-based rehabilitation service provides a viable alternative to Tier 4 inpatient facilities, allowing individuals to thrive in a secure and familiar environment at home with their families.
Learning disabilities and autism
We provide personalised clinical and behavioural support to individuals with learning disabilities and/or autism, tailoring care from 6:1 to 1:1 or individual hours to suit their needs; placing a strong emphasis on Positive Behavioural Support (PBS) promoting personal choice and inclusion.
All of our healthcare professionals receive expert physical intervention training, leading to a reduction in behaviours of concern and restrictive practices for the individuals we support.
