Supporting patients in their home reduces waiting lists, avoids unnecessary hospital admissions and improves accessibility for service users who can be seen at a time that suits them.

We work in partnership with primary and secondary care to optimise patient pathways, working alongside existing services to increase capacity and effectiveness. Many of our services are CQC registered, and in all cases, we work closely with our partners to align clinical governance and to deliver services safely.
